Clinicians are invited to join us only when there is a sufficient number of clients to support additional therapists. We are currently accepting applications beginning September 1, 2006.

To apply, please submit the following:

1.) a copy of your current valid state license(s)/certfication(s)

2.) verification of malpractice insurance coverage

3.) verification of advanced training in anxiety disorders

4.) submit the following Find-a-Therapist Network Provider Form

5.) submit $95.00 one time application processing fee. Use the 'Buy Now' Button to pay online by credit card or check.  Fax your credentials to 480-396-3213.

Upon acceptance into the Network, you receive a provider agreement which establishes your relationship with Find-a-Therapist, Inc. as an Independent Contractor.  You will be billed a one time Virtual Office Set Up Fee of $195.00.  There are no other charges required to be part of our Network.

Your Virtual Office includes Secure Chat capabilities, Online Appointment Scheduling, Credit Card processing and secure email accounts. You receive a complete personal orientation by phone and online to our policies and procedures.

Support services are available if you need assistance of any kind.

We manage advertising, promotional campaigns, search engine submissions and highlighting your online services once you are accepted into our provider network.  

Everything you need to begin practicing online is included here. There are no ongoing or monthly fees.
